Google Forms is free, unlimited, and already inside every school and workplace Google account. For a quick survey or a simple sign-up, it is the right answer and nothing here beats free-and-already-installed.
The actual difference
Google Forms asks you to type every question. For a five-question survey that is a couple of minutes. For a two-page intake form or a permission slip with a medical section, it is an evening — and at the end you have a Google Form that looks nothing like the document your organisation approved.
Three things Google Forms does not do at all: it will not read an existing document, it has no real signature field, and it cannot give you the answers back as your original PDF with the boxes filled in. If any of those three matter, the comparison ends there.
If none of them matter, use Google Forms. We would.
