What a tattoo consent form is protecting
A tattoo consent form does three jobs at once. It records that the client disclosed health conditions that affect healing or bleeding. It records that they understood the risks and the permanence. And it records that they were sober enough to consent.
Most health departments require the form to be dated the day of the appointment, which matters more than it sounds: a client's medications, health and state of mind can all change between booking and sitting in the chair.
The health questions that actually matter
Bleeding disorders and blood thinners change how the session goes. Diabetes and immune conditions change how it heals. Epilepsy changes what you do if something goes wrong. Pregnancy is a common reason to postpone. Antibiotics and steroids affect healing.
Ask them as a checklist rather than an open box. A checklist gets answered; "any medical conditions?" gets left blank by people who did not think theirs counted.
Sobriety is a consent question, not a formality
Consent given under the influence is not reliable consent, and most jurisdictions expect a studio to refuse. Having it as an explicit signed line protects the artist and gives the front desk a reason to point at when they turn somebody away.
This is the form paper is worst at
A consent form is signed in person, minutes before the appointment, by somebody who is already sitting down — and then it has to be kept, findable, for years, because the whole point is being able to produce it later.
A tablet at the desk does that better than a filing cabinet in every respect: it is dated automatically, it cannot be filled in illegibly, it cannot be lost, and it is searchable when somebody asks for a copy in three years. Photographs of the work need their own permission
Consent to be tattooed is not consent to appear on the studio's account. They are different permissions, and combining them into one signature means neither is clearly given — a client who wanted the tattoo and not the photograph has no way to say so.
Ask separately, and make the photography question optional. If the images will be used in advertising rather than only posted, that is a model release rather than a consent line, and it belongs on its own form.
