A field trip slip needs more than a general permission slip

A generic permission slip covers consent. A field trip adds three things a general one does not: the child leaves the site, they travel in a vehicle, and they are away over a mealtime.

So a trip slip needs transport consent as its own question, departure and return times so parents know when to collect, and lunch arrangements so nobody arrives without food.

Ask about medication separately from medical needs

"Any medical needs?" and "will your child be carrying medication?" are different questions and the second one matters more on a trip. Staff need to know before departure who has an inhaler in their bag and who has an EpiPen, and where it is.

Ask it explicitly. A note in a free-text box gets read once, in the office, a week before.

The blank-cell problem

If a parent leaves the medical question blank, does that mean no medical needs, or that they did not read it? On paper you cannot tell, and the difference matters when the question is about an EpiPen.

Requiring an explicit yes or no removes the ambiguity. The online version of this template will not submit until the question is answered, so an empty answer never reaches the teacher who is packing the bus.

The trip leader needs it on paper, on the day

Everything gained by collecting slips as a link is lost if the only copy is on a phone in a coach park with no signal. The list of emergency contacts, allergies and medication is the one document that has to work when nothing else does.

So print it before you leave, one page, sorted by name, and give a copy to every adult supervising. Not the slips themselves — a single sheet with the pupils on it and the four things that matter beside each name. Whoever ends up standing with a child at a first aid point is the person who needs it, and it will not be whoever set the form up.

Take a second copy that stays at school. If a parent has to be reached and the trip leader is unreachable, somebody in the office needs the same list.

Count the adults before you count the children

Supervision ratios are usually set by the school or the local authority rather than by the trip, and they are the thing that cancels a trip late. A slip that also asks parents whether they are available to help gives you the number early, while there is still time to do something about it.

Volunteering parents need their own record: name, phone number, whether they are driving, and whether their clearance is current. That is not the same set of answers as a permission slip and it is worth a second short form rather than extra questions on the first, which every parent then has to skip.

Use the slip your school already approved

Most schools and districts have a permission slip whose wording has been through somebody's legal review. There is no reason to rewrite it — upload the PDF or photograph the paper version and the questions come off it as they are, so the approved wording is preserved and it becomes a link.
