Put the regrowth myth on the form
Every dermaplaning client has heard that the hair grows back thicker. It does not — vellus hair is cut at the surface and regrows at the same texture — but the belief is universal enough that it is worth a signed line rather than a reassurance in conversation.
Having it on the form does two things: it settles the question before treatment, and it means a client who comes back convinced otherwise is looking at their own signature on the sentence.
Actives are the screening answer that changes today's plan
Skin that is still on tretinoin or an acid active is thinner and more prone to abrasion. Asking "when did you stop?" rather than "do you use retinol?" gets the answer that actually decides whether to proceed and how lightly to work.
Isotretinoin within six months, active or cystic acne in the area, and open or broken skin are the three answers that mean rebooking.
Record what else is happening in the same appointment
Dermaplaning is usually one part of a longer treatment, and a peel or enzyme applied to freshly dermaplaned skin behaves differently from the same product on untreated skin. Recording the combination is what makes the note useful if the client reacts.
Expectations, in three lines rather than one
Tightness and flushing for a day or two, the possibility of a small nick, and the possibility of a breakout afterwards are the three things clients report as surprises. Each is ordinary; each generates a message to the front desk when it was not mentioned first.
One form, signed before every appointment
Dermaplaning is a repeat treatment on a four to six week cycle, which makes it the clearest case for a link rather than a clipboard: the client answers the actives question honestly at home, the record is dated automatically, and nothing has to be reprinted.

What should a dermaplaning consent form include?
Client details and treatment date, whether it is a first treatment, anything else being done in the same appointment, screening for isotretinoin, active acne, cold sores, broken skin, recent peels and blood thinners, when retinol or acids were last used, allergies, an explicit statement that hair does not grow back thicker, the expected tightness and flushing, the risks, sun protection, aftercare and a dated signature.
Does dermaplaning make hair grow back thicker?
No. Vellus hair is cut at the surface and regrows at the same texture and colour. It is the most common client belief about the treatment, which is why this template puts it in the consent rather than leaving it to conversation.
Who should not have dermaplaning?
Isotretinoin use in the last six months, active or cystic acne in the treatment area, cold sores, sunburn, open or broken skin, and a recent peel or laser are the standard reasons to postpone.
How often can a client have dermaplaning?
Most practices work on a four to six week cycle. Because it repeats that often, taking consent by link rather than on paper saves the reprinting and gives you a dated record each time.
